DECLARATION OF ELIGIBILITY FORM
NOTE: This also lets the school district know your kiddo will be out of class and gives them the opportunity to grant an excused absence. We have not had difficulties with school districts giving an excused absence in the past.
However, it is at their discretion whether they grant one or not.
- Download the Eligibility Form (click here)
- Fill out the lines highlighted in yellow (including the one at the very bottom of the page)
- Email the form to [email protected] at least a week prior to the event
- You will be copied on the email sent to the school
- 4-H follows the same UIL rules for eligibility to participate in an off campus activity
